Part-Time Admission Registered Nurse (RN)
Take charge of the start of care journey. As an RN Admission Specialist with Home Health Care, Inc., you’ll deliver expert OASIS documentation and launch personalized care plans for clients transitioning into home-based care.

- Solo Visits: Deliver direct care in the field with freedom and purpose
- Client-Centered: Build trust as the first clinician in the care journey
Responsibilities:
- Perform comprehensive patient assessments and OASIS documentation
- Recommend and coordinate additional services
- Communicate with providers and caregivers to ensure safe transitions
Qualifications:
- Minnesota RN license in good standing
- Admission and OASIS experience
- Strong communication and time management
- Reliable transportation
Compensation: $150 - $200 per visit + mileage reimbursement!
