We are seeking a compassionate and organized Social Services Assistant to join our team at Roswell Nursing & Rehabilitation Center. As a Social Services Assistant, you will play a vital role in ensuring that our residents receive exceptional care, support, and services that meet their individual needs. • Assist the Social Services Manager in coordinating and implementing social services for residents, including but not limited to assessments, care plans, and service delivery.
• Provide emotional support and counseling to residents and their families to address their psychosocial needs.
• Facilitate resident activities and programs to enhance their quality of life and promote socialization.
• Collaborate with other interdisciplinary team members to develop and implement care plans that meet resident needs.
• Maintain accurate and up‑to‑date records of resident information, assessments, and service delivery.
• Participate in quality improvement initiatives to improve social services and overall resident care.

• High school diploma or equivalent required; degree in social work, counseling, or a related field preferred.
• Strong communication and interpersonal skills with the ability to work effectively with residents, families, and staff.
• Ability to maintain confidentiality and handle sensitive information with discretion.
• Familiarity with guidelines and regulations governing long‑term care and rehabilitation services.
