• Take responsibility for areas of chronic disease management within the practice.
• Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
• Provide primary support to general practice staff with prescription and medication queries.
• Support the repeat prescription system, manage acute prescription requests, and conduct medicines reconciliation on transfer of care.
• Implement safer prescribing systems and provide expert clinical medicines advice covering public and social care needs.
• Integrate practice operations with community and hospital pharmacy to optimise skill mix and improve patient outcomes.
• Contribute to operational efficiencies and quality of care by delivering excellent service within general practice.

CONNECT HEALTHCARE ROTHERHAM CIC is a federation of 28 GP practices in Rotherham, led by a Board of representatives and a part‑time Medical Director. The pharmacist will support Doncaster East PCN.
