As a Care Coordinator, you’ll be the key link between clients, families, and care workers. Reporting to the Branch Manager, you’ll ensure care is delivered smoothly, safely, and consistently — while supporting and developing our frontline teams.
YourKey Responsibilities
- Create and manage staff rotas, ensuring appropriate travel time and breaks
- Match carers to clients based on skills, preferences, and location
- Manage new referrals from private clients and social services
- Support assessments, care planning, and care package reviews
- Communicate with clients, families, and healthcare professionals
- Carry out risk assessments, reviews, and care worker supervisions
- Ensure care documentation and client records are accurate and up to date
- Support weekly KPIs, logbook drops, and team meetings
- Organise and participate in supervisions, appraisals, and training
- Take part in the on-call rota, including alternate weekends
- Provide occasional support to neighbouring branches when required
