The position of Advanced Nurse Practitioner requires the post holder to act within professional boundaries to provide comprehensive patient care, including history taking, clinical assessment, diagnosis, treatment and evaluation. The role demands safe clinical decision‑making, expert care, critical thinking, and collaborative teamwork with the general practice team to support policy delivery and nurse leadership. Advanced practice NMC registration is mandatory to work at this level.
Key Responsibilities- Provide full patient assessment and management from history taking to treatment and evaluation.
- Demonstrate safe clinical decision‑making and expert care with advanced assessment and diagnostic skills.
- Apply critical thinking in the clinical decision‑making process.
- Collaborate with the general practice team to meet patient needs and support delivery of policy and procedures.
- Provide nurse leadership as required.
- Currently employed or self‑employed as an advanced level nurse in accordance with the four pillars of advanced practice.
- MSc or Masters in Advanced Practice, or equivalent.
- Relevant nursing or health degree with mentoring/teaching qualification.
- Clinical supervision training.
- Qualification in long‑term conditions management.
- Two examples of CPD evidence related to Advanced Clinical Practice within the previous three years.
- Working autonomously in an advanced nursing role.
- Audit and research nurse prescribing experience.
- Involvement with community development initiatives.
- Health needs assessment experience.
- Experience working within primary care or General Practice.
- Case management experience.
- Nurse‑led triage experience.
- Advanced clinical practice skills.
- Management of patients with long‑term and complex needs.
- Clinical examination skills.
- Accountability of own and other roles in nurse‑led services.
- Local and national health policy.
- Clinical governance issues in primary care.
- Public health knowledge and determinants of health.
- Clinical leadership.
- Written and verbal communication.
- Communication of difficult messages to patients and families.
- Negotiation and conflict management.
- Change management.
- Teaching and mentorship in a clinical setting.
- Resource management.
- Proficiency with Systm One.
- Must hold appropriate UK professional registration.
- Position subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 – Disclosure to the Disclosure and Barring Service will be required.
