• Assessment & Planning: Complete Care Act assessments, reviews, and person-centred support plans.
• Mental Capacity: Conduct Mental Capacity Assessments (MCA) and Best Interest Decisions (BID).
• Safeguarding: Identify and respond to safeguarding concerns and manage complex risk situations.
• Multi-Agency Working: Work collaboratively with families, carers, and partner agencies to provide holistic support.
• Documentation: Maintain accurate records and reports using systems such as LAS.
• Transition Support: Ensure seamless transitions that promote independence and positive life outcomes.
