Overview
The service triages and assesses all urgent and emergency referrals to identify the most appropriate service for individuals experiencing a mental health crisis. The team currently consists of Community Mental Health Nurses, Psychologists, a Consultant Psychiatrist and other associated medical staff. Service provision is maintained 24/7 over a varied shift pattern.
Responsibilities- Provide a clinical service that is service‑user focused.
- Triage all mental health referrals to the Crisis Team to determine priority for service response and the appropriate clinical pathway.
- Carry out full assessments of needs, including comprehensive risk assessments of persons referred or self‑referenced to the service.
- Recommend and refer, where treatment/intervention is indicated, to the most appropriate team or service utilising all available resources.
- Identify appropriate treatment‑based interventions for all service users, ensuring that best practice is implemented.
