An exciting opportunity has arisen as an Assessment & Review Co-Ordinator to work with our Localities Adult Social Care Teams!

North, Central, and South
areas of the borough have a base in the town centre and a regional office in Maltby.

Our Locality Teams work with adults of all ages, who are experiencing the impacts of short- or long-term health conditions, physical and/or cognitive impairments. We work holistically to support carers and families, valuing the support they provide too.

As well as being part of a supportive team, you willbenefitfrom regularprofessionalsupervision,as well as an extensive training and development program.

About you

  • Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, Making Safeguarding Personal, and Continuing Healthcare Frameworks.
  • Root your practice in compassion, values and the understanding of thewhy of what we do in our unique profession, to best promote people’s wellbeing.
  • Be confident in working with situations of varying complexity and be able to make appropriate decisions quickly to support those in crisis, and to ensure people are seen and supported in a timely manner.
  • Know how to work with our partner agencies to help people find solutions to maximise their independence and resilience, enabling people to take positive risks, and lead good lives.
