An exciting opportunity has become available for a Team Manager in the Preparing for Adulthood team in Rotherham Adult Social Care. At Rotherham, we believe that every young person should be supported to move into adulthood with ambition, purpose and opportunity. We are looking for an exceptional Team Manager to lead our Preparing for Adulthood Team, driving high-quality transition planning and ensuring young people and their families experience a seamless journey into adult life.

This is an exciting opportunity to manage a specialist team at the heart of our partnership between Children's and Adult Services. You will champion a culture where aspirations come first, where young people are supported to achieve independence, develop meaningful relationships, access employment and education opportunities, and build lives connected to their communities.

About the Team Our Preparing for Adulthood Team works with young people from the age of 14 onwards who are likely to require support from Adult Social Care.

The team has a unique and influential role in supporting young people, families and professionals to plan positively for adulthood and ensure that transitions are proactive, coordinated and focused on outcomes rather than services.

About the Role You will provide leadership to a skilled and committed team, creating an environment where professional curiosity, innovation and strengths-based practice thrive. You will support practitioners to work creatively with young people and families, ensuring plans are built around aspirations, outcomes and opportunities rather than limitations.

You will play a key role in strengthening partnerships across Children's Social Care, SEND Services, Education, Health and Adult Social Care, helping to remove barriers and create pathways that support young people to flourish as adults.

About You You will be a registered Social Worker with substantial experience within Adult Social Care and a strong understanding of Children’s legislation and transition practice.

You will bring:
