Een zorgorganisatie in Rotterdam zoekt een Medewerker Individuele Ondersteuner Thuis voor 24 uur per week. In deze rol begeleid je zelfstandig wonende cliënten met complexe zorgvragen en stel je begeleidingsplannen op in samenspraak met de cliënt. Je werkt nauw samen met andere professionals en betrekt het informele netwerk van de cliënt. Deze functie biedt ruimte voor persoonlijke en professionele ontwikkeling met goede arbeidsvoorwaarden conform cao VVT.
