Zorg is op zoek naar een casemanager dementie in Hoogvliet voor 24 uur per week, per 1 november 2026. In deze rol ben je verantwoordelijk voor het begeleiden van cliënten met dementie en hun netwerk.
Je coördineert de zorg, schakelt tussen betrokken partijen en draagt bij aan het welzijn van de cliënt. De functie biedt flexibiliteit in werktijden en een collegiaal team waarin je zelfstandig kunt werken.
