RIOzorg zoekt een casemanager die werkt in Rotterdam en meebouwt aan onze toekomstige locatie in Voorne aan Zee (Hellevoetsluis). Het merendeel van de week werk je in Rotterdam, één dag op locatie in Hellevoetsluis met uitbreiding mogelijk.
Je voert behandel- en diagnostische trajecten uit bij kinderen en jongeren tot 18 jaar, bepaalt behandelplannen en monitort voortgang, in nauwe samenwerking met je regiebehandelaar en wijkteams.
