De Skala is een dynamische zorglocatie in Rotterdam-Zuid waar professionals cliënten met verslavingsproblematiek en GGZ-problematiek begeleiden. Je werkt zelfstandig aan een eigen caseload, voert cliëntgesprekken en stelt cliëntplannen op in nauw overleg met behandelaren en het multidisciplinaire team.
De functie vraagt een relevant zorg- of welzijnsdiploma en ervaring binnen GGZ/verslavingszorg. Flexibiliteit in dag- en nachtdiensten is gewenst; verdere professionele ontwikkeling wordt
