Zeno Zorg zoekt een coördinerend begeleider voor ambulante zorgtrajecten in de Rotterdamse omgeving. Je werkt zelfstandig mét het team aan structuur, regie en zelfstandigheid van cliënten, met oog voor wat wél werkt.
Je ziet wat er speelt bij (jong) volwassenen binnen de Wet langdurige zorg en bewaakt het hele traject, inclusief evaluaties, roosterafstemming en verslaglegging. Een rijbewijs en minimaal drie jaar relevante ervaring zijn vereist.
