MEE Rotterdam Rijnmond zoekt een Medewerker Wlz-expertiseteam om een nieuw expertiseteam op te zetten en dit intern en extern te positioneren. Je onderhoudt relaties met CIZ en ontwikkelt het afwegingskader Wlz.
Je onderzoekt routes naar onafhankelijke cliëntondersteuning bij diverse gemeenten en werkt zowel uitvoerend als ondersteunend voor professionals. Ook zet je een overlegstructuur op met het Toegang- en Regieteam en beoordeelt Wlz-aanvragen en indicaties.
