Stichting Antes zoekt een HBO-verpleegkundige of agog voor het GGZ-team Prinsenland in Rotterdam. Je werkt zelfstandig in ambulante zorg binnen een multidisciplinair team en behandelt cliënten thuis of op de poli.
Je hebt een BIG-registratie of Social Work-opleiding, ervaring met EPA is een pré. Je draagt bij aan herstelgerichte zorg, voert intakegesprekken en bewaakt de zorgdossiervoering.
