In deze rol bied je begeleiding aan en werk je samen met andere zorgprofessionals, waarbij je de focus legt op de eigen regie van de cliënt en de kracht van hun netwerk. Je werkt volgens bewezen methoden zoals oplossingsgericht werken en sociale netwerkversterking.
