Kessler Perspektief zoekt een Trajectondersteuner die cliënten begeleidt op basis van hun ondersteuningsvragen en doelen. Zelfstandigheid en eigen regie staan centraal, met samenwerking met ketenpartners en mogelijk betrokken netwerk. Er wordt gewerkt aan variabele dagdiensten, inclusief avond-, nacht- en weekenddiensten.
Je hebt een MBO-4 diploma Sociaal Werk en affiniteit met de doelgroep. Flexibiliteit in werktijden, een dosis humor en gastvrijheid zijn gewenst.
