ASVZ zoekt een betrokken persoonlijk begeleider voor 7 bewoners op Doggersbank 47D. Je coördineert behandeling en begeleiding volgens Triple-C, en werkt mee in directe zorg. Je betrekt verwanten, teamleaders en orthopedagoog bij planning en overleg.
Je hebt een afgeronde SPH/MMZ 4, SPW 4 of MBO 4, of AD SPH/Social Work. Je toont sensitief handelen, doorzettingsvermogen en verantwoordelijkheid en werkt onregelmatige diensten.
