ASVZ zoekt een betrokken persoonlijk begeleider voor een groep bewoners met MVB/LVB in Rotterdam. Je coördineert behandeling en begeleiding en voert zelf uitvoering uit in directe begeleiding van cliënten, met aandacht voor dagelijkse activiteiten, ADL en betrokken zorgverlening.
Je werkt nauw samen met een orthopedagoog en teamleider, bepaalt samen met hen de planning en neemt deel aan werkoverleg. Onregelmatige diensten behoren tot de werktijden.
