Humanitas DMH zoekt een persoonlijk begeleider voor een tijdelijke woonlocatie in Rotterdam Hoogvliet. Je werkt altijd in duo en biedt nabijheid, structuur en signalering van veranderingen. Een mbo-4 zorgopleiding is vereist en je handelt met geduld en empathie.
Je voert ADL-ondersteuning uit en helpt bewoners hun eigen regie te vergroten. Onregelmatige uren zijn mogelijk, maar slaap- en nachtdiensten zijn niet gevraagd. Ervaring met bewoners met beperking is een pluspunt.
