Koninklijke Visio zoekt een ervaren Ambulant Specialistisch Begeleider voor de regio Rotterdam. Je werkt samen met zorgprofessionals om cliënten te ondersteunen bij het behouden van eigen regie in de thuissituatie, met aandacht voor visuele beperking en bredere leefgebieden.
Je bezoekt cliënten regionaal en werkt waar mogelijk vanuit huis. De functie is gericht op langdurige begeleiding en samenwerking met gemeenten, volgens het gedachtegoed van positieve gezondheid.
