Kaap 38 in Rotterdam zoekt een persoonlijk begeleider die een woongroep van negen bewoners ondersteunt. Je helpt bij het ochtendritueel, begeleidt bewoners bij dagbesteding en activiteiten en bent mentor voor een aantal bewoners.
Je werkt samen met collega’s om structureel zorg te bieden en ontwikkelt ondersteuningsplannen. Je hebt een afgeronde MMZ4/Verpleegkundige niv4/SPW4/SPH en bent toegewijd aan de visie van ASVZ.
