Zorg zoekt een betrokken ambulant Begeleider om jonge deelnemers (18–31 jaar) te helpen herstellen en zelfstandig verder te gaan. Je werkt in een klein team binnen Cité en bezoekt deelnemers thuis, gericht op maatwerk en professionele nabijheid.
Je hebt een HBO-opleiding Social Work of vergelijkbaar, met praktische ervaring en een sterke focus op verbinding en veerkracht. Een VOG is vereist en de functie biedt een jaarcontract met uitzicht op verlenging, inclusief uitstekende
