Aafje is op zoek naar een Medewerker Individuele Ondersteuner Thuis die zelfstandig wonende cliënten in Rotterdam Noord en Drechtsteden helpt met complexe zorg- en begeleidingsvragen op basis van de WMO.
Je werkt 24 uur per week, legt verantwoording af aan de Teammanager IOT en bouwt aan een persoonlijk begeleidingsplan, gericht op zelfredzaamheid, regie en structuur. Een eigen vervoer is noodzakelijk en je werkt in netwerkverband met professionals en het informele netwerk.
