ASVZ zoekt een jeugd- en gezinsprofessional die gezinnen thuis ondersteunt bij opvoeding en ontwikkeling. Je werkt met gezinnen met uitdagingen zoals verstandelijke beperking en psychische problematiek, gebaseerd op Houvast en Triple-C.
Je verzorgd behandelplannen en samenwerking met school, wijkteams en instanties, met ruimte voor eigen regie en rapportage aan het eind van elke dag. Je hebt een HBO-opleiding in SPH/J/Pedagogiek/MWD/Toegepaste Psychologie en bij voorkeur SKJ-registratie.
