Called to serve patients in their homes, the Nurse Practitioner approaches each day with a passionate commitment to delivering personalized, one-on-one care—helping patients regain their independence and achieve their health goals. In this role, you will provide hospice care through the evaluation and assessment of the overall medical conditions of patients, complete face-to-face assessments and evaluate hospice eligibility.

As The Nurse Practitioner, You Will- Provide medical care, palliate, and manage terminal illness.
- Complete history and physical examinations, evaluating the patient’s medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting findings.
- Develop individualized plan of care, prescribe medication and/or treatments, obtain consultations.
- Complete hospice face-to-face visit requirements.
- Review and develop protocols for treatment, and consult the collaborating physician when the patient’s plan of care is outside standardized practice and protocols.
- Excellent interpersonal, organizational, and problem‑solving skills.
- Ability to meet deadlines and patient needs with diligence.
- Demonstrate proven decision‑making skills.
- Familiar and comfortable with technology. HCHB experience is a plus.
- Licensed Nurse Practitioner with the State Nurse Practice Act Licenses in the state of operations where assigned branch is located.
- Minimum of one year of Nurse Practitioner experience.
- Minimum of six months to one year of experience in hospice or pain and symptom management.
- Reliable transportation and valid and current driver’s license.
