FONCTIONS ET RESPONSABILITÉS
Élaborer et réviser des plans de soins infirmiers individualisés pour chaque patient(e);
Effectuer des évaluations de l'état de santé des patient(e) s et enseigner les procédures d'administration des médicaments;
Obtenir un accès intraveineux par le biais de la mise en place d'une perfusion intraveineuse périphérique ou d’un accès au cathéter central (CCIP, DAVC, etc.);
Préparer différents traitements pharmacologiques spécialisés et les administrer par voie intraveineuse ou par injection;
Fournir des soins aux patient(e) s en utilisant les compétences d’observation, d’évaluation, de diagnostic infirmier, de conseil et d’enseignement sanitaire, et signaler tout événement indésirable au (à la) superviseur(e) direct(e) ou aux parties prenantes requises;
Effectuer les tâches infirmières courantes conformément aux politiques et aux procédures de l’entreprise ainsi qu’aux normes de l’organisme provincial de réglementation des soins infirmiers;
Fournir des soins centrés sur le (la) patient(e) conformément au plan de soins établi ou aux directives médicales;
Effectuer des interventions de soins infirmiers spécialisés impliquant une formation reconnue et des compétences actuelles qui ont été démontrées et actualisées conformément aux normes;
Poursuivre, selon les besoins, les formations liées au rôle, aux médicaments et à la pharmacovigilance;
Observer et documenter l’état de santé des patient(e) s;
Signaler et documenter les événements indésirables conformément aux exigences de la pharmacovigilance;
Accomplir toutes les autres tâches demandées.
JOB SUMMARY / SOMMAIRE DU POSTEThe Specialty Pharma Nurse - Registered Nurse, under the direction of the Specialty Pharma Supervisor and/or Specialty Pharma Nurse Manager, delivers and monitors specialized infusion, injectable or oral pharmacological treatments in the home and/or clinic setting. The infusion nurse is responsible for monitoring the patient for infusion related reactions (intervening as required), accurately documenting the procedure, and submitting reporting in a timely manner.
The Specialty Pharma Nurse - Registered Nurse is expected to maintain effective communication with the Specialty Nursing Supervisor and/or Specialty Pharma Nurse Manager to ensure patient safety and satisfaction. Specialized pharmacological training will be provided. This career will allow you to develop a specialty in infusion services with opportunities to attend conferences and participate in clinical trials. The position is casual in nature as the nurse will provide their availability and manage their patient portfolio. Bayshore will work with you to assemble a working schedule that fits your lifestyle. Specialty Pharma Nursing is a one-on-one patient experience, and we strive to ensure that patients stay with the same nurse infusion to infusion to ensure the best quality of care.
AND RESPONSIBILITIES / RESPONSABILITÉS
Develop and review nursing care plans that are individualized to each patient.
Conducts patient health assessments and medication administration teaching.
Obtain IV access through peripheral IV initiation or central line access (PICC, IVAD, etc.).
Prepares and administers a variety of specialized intravenous or injectable pharmacological treatments.
Provide patient care by utilizing the skills of observation, assessment, nursing diagnosis, counseling, health teaching, and reporting any adverse events to immediate supervisor or required stakeholders.
Perform routine nursing duties in accordance with company policies and procedures, and provincial body standards.
Provide patient centered care according to the established care plan or medical directive.
Perform specialized nursing procedures for which recognized training and current competencies have been demonstrated and updated per standards.
Maintain role related, medication and pharmacovigilance training as required.
Observe and document patient conditions.
Report and document Adverse Events as per Pharmacovigilance requirements.
