Join West Kent Primary Care andbecome a vital member of the Mote & Wallis Avenue Health Primary Care Network team as a PCNClinical Pharmacist. You will join an existing clinic pharmacy team of 1 clinical pharmacist and 1 pharmacy technician. We are looking for a further clinical pharmacist who is passionate about developing their skills in General Practice.

This role will support The Mote Medical Practice and Wallis Avenue Surgery in expanding capacity for patients to access a clinical professional. The practices have a combined patient population of around 17,000. The post holder will work across three sitesalongside the PCN Lead Pharmacist and PCN Pharmacy Technician. The PCN also provides MSK and Childrens Social Prescribing services.

Main duties of the jobThe pharmacists will develop, enable, and support a cultureof multidisciplinary care with a focus around developing PCN services in linewith national and local specifications. Working directly with various practice teams; this role will directly link to colleagues in community and acute settings ensuring patient health and outcomes are met in a collaborative and innovative environment.
The role will require a commitment to register on the CPPE pathway and Independent Prescribers Course (if these havent previously been completed), which we will fully support you in by providing you with time to attend training sessions plus a GP clinical supervisor and peer support.
You will collaborate closely with GPs and other Healthcare Professionals, conducting patient consultationsboth via telephone and face to face, assisting with care plans for long term condition management (including hypertension, cardiovascular disease, asthma,diabetes and more), updating records, ensuring patient safety and managing patients within the appropriate care setting plus undertaking audits to ensure patient medication safety.

* Due to the geographical location, it would be beneficial to have a full driving license as travel will be required to practices across the PCN area.
Job responsibilities- Work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing.
- Perform face-to-face medication reviews of patients with polypharmacy, especially for those with frailty and/or with multiple co-morbidities and prescribe alternative medications.
- Undertake face-to-face reviews of patients with specific long-term conditions that fall within their competency (such as asthma reviews).
- Carry out Improved Access and Extended Hours sessions, including some weekend work.
- Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
- Contribute to practice financial stability through optimisation of medicines related targets in QOF delivery.
- University Degree in Pharmacy Sciences
- Registered with the General Pharmaceutical Council
- Registered Prescriber
- Experience in providing clinical advice and expertise on medicines to colleagues and patients
- Previous experience as a community, hospital or primary care based pharmacist
- Experience using MS Outlook and Calendar management.
- Experience of managing long-term conditions in a clinical setting
- Able to use Microsoft Office applications with above average capability with Word, Excel and PowerPoint.
