Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

About the role

Are you a compassionate and motivated Band 6 Nurse ready to make a real difference in your community? Join our dynamic Community Nursing team, delivering high-quality, personalised care to housebound adults with complex and long-term conditions.

Key Responsibilities
  • Support patients to remain safe and well in their own homes, helping to prevent unnecessary hospital admissions.
  • Work within a Primary Care Network and multidisciplinary team, managing a diverse caseload.
  • Deliver holistic assessments, care planning, and clinical interventions including wound care, IV therapy, palliative care, and chronic condition management.
  • Coordinate care delivery, triage referrals, and support junior staff through supervision and mentorship.
  • Use clinical expertise to assess complex needs, make informed decisions, and promote independence through patient education.
  • Contribute to service improvement, quality standards, and multidisciplinary meetings, ensuring the highest levels of patient care.

  • Opportunity to progress towards (or complete) the Specialist Practitioner District Nurse qualification.
