All screens Nationwide Ltd. in Tonbridge is seeking an Advanced Clinical Practitioner to join the Tonbridge Primary Care Network’s Same Day Access Hub. You will work with GP supervisors and the Frailty Visiting Team to provide urgent, on‑the‑day care for diverse patient needs in a multidisciplinary setting.
You will triage, assess and manage acute and complex conditions, prescribe within scope, coordinate referrals and ensure safeguarding and patient safety in line with current guidelines.
