Practice Nurse – Diabetes Lead

An excellent opportunity has arisen for an experienced Practice Nurse to join a well‑established and supportive GP practice in the Tunbridge Wells area.
The practice is looking for a confident and experienced Practice Nurse who has a particular interest in Diabetes management, or someone who is keen to develop into a specialist Diabetes‑focused role. This is an exciting opportunity to take a lead role in the delivery and development of diabetic care within the practice.
You will join a friendly multidisciplinary team and have the opportunity to work autonomously across a varied Practice Nurse remit, while developing a key area of clinical responsibility.
The RoleThe successful candidate will provide high‑quality nursing care to patients across a broad range of primary care services, with a particular focus on Diabetes management and long‑term condition care.
You will be responsible for assessing patients, identifying clinical abnormalities, developing individualised management plans and providing ongoing monitoring and education.
There is scope for the successful candidate to become the practice's key clinician for Diabetes care, making this an excellent opportunity for a nurse looking to develop their clinical responsibilities.
Key ResponsibilitiesLead and develop Diabetes care within the practice
Chronic disease management and reviews
Therapeutic monitoring and medication compliance
Patient health checks and health promotion
Wound assessment and management
Risk assessment and appropriate referrals
Health screening and preventative care
Cervical screening and women's health
Travel health
Adult and childhood immunisations
First‑contact patient assessment
Respiratory management
Supporting patients with mental health needs
Assisting with minor procedures
Working closely with GPs and the wider multidisciplinary team
Contributing to clinical audits, quality improvement and practice development
Permanent part‑time position

We are looking for an experienced Practice Nurse who is confident working within general practice and passionate about delivering high‑quality patient‑centred care.
You will ideally have:
Current NMC registration
Previous Practice Nurse/general practice experience
Experience managing long‑term conditions
A strong interest in Diabetes management
Excellent clinical assessment and communication skills
The ability to work autonomously
A proactive and patient‑focused approach
A willingness to develop and take ownership of Diabetes services
Diabetes experience is highly desirable, although candidates who are keen to specialise in this area and have the appropriate primary care experience will also be considered.
