An exciting opportunity has arisen within the Heart, Lung, and Critical Care Clinical Group at Harefield Hospital for an experienced and innovative nurse to join our team, which is led by the Consultant Nurse in Cardiology.
This clinical role will enable you to develop your skills in a variety of aspects of cardiology nursing, including pre‑admission clinics for elective procedures and outpatient management of patients following elective and Primary PCI. The role may include nurse‑led discharge of patients from the day‑case unit.
The successful candidate will need to be highly self‑motivated, be able to demonstrate advanced management, communication, interpersonal and organisational skills and be able to work autonomously as well as within the Cardiology team. You will receive in‑house training and study leave to help you develop your potential. Previous experience in a CNS role related to managing patients with coronary disease is required.
Qualification in clinical assessment and as a non‑medical prescriber would be advantageous but training can be given.
- Be a highly specialised nurse working with a specified group of cardiology patients and taking responsibility for their ongoing clinical management.
- In collaboration with the Consultant Nurse in Cardiology, manage the continuing care of patients who are undergoing PCI.
- Deliver a high standard of care to cardiology patients.
- Run nurse‑led pre‑admission clinics.
- Run nurse‑led follow‑up clinics for patients post‑PCI.
- Optimise secondary prevention.
- Act as a resource for advice on management of patients with coronary artery disease.
- Manage the outpatient follow‑up care for patients who have undergone PCI, including assessment, planning, implementation and evaluation.
- Arrange admission dates for procedures and follow‑up appointments as per PCI follow‑up protocol.
- Use expert judgment and analytical and problem‑solving skills in the surveillance and recognition of changes in the patients’ condition, referring to senior/medical staff as necessary.
- Independently manage a caseload of patients and associated responsibilities.
- Ensure accuracy of documentation and maintain an efficient record system in agreement with the cardiology team.
- Be aware of the legal implications of all patient‑related records.
- Maintain the BCIS/CCAD patient database.
- Assist with the running of pre‑admission clinics for all elective cardiology admissions.
- Participate in nurse‑led discharge.
- Manage the psychological issues related to caring for patients on a long‑term basis.
- Assist with the running of other cardiology nurse‑led services.
- Act as an expert resource for patients, relatives and colleagues.
- Maintain patient confidentiality, privacy, dignity, religious and cultural needs at all times.
- Be an independent non‑medical prescriber and prescribe within scope of practice /area of competency.
- Follow locally approved clinical guidelines, NICE and the formulary.
- Provide cost‑effective prescribing activity and regularly review prescribing activity and implement change as necessary.
- Have prescribing activity audited annually and work within the Trust Non‑Medical Prescribing Protocol.
- Interpret blood results, ECGs and other non‑invasive tests and act accordingly.
- Maintain an environment for safe and effective patient care in accordance with the Trust Risk Management Policy.
- Work effectively within the Trust's adult multidisciplinary cardiology team, ensuring an individualised, evidence‑based optimum service for each patient and their family.
- Act as advocate for cardiology patients.
- Promote collaborative relationships with other health‑care professionals for quality patient care.
- Maintain effective communication channels within the multidisciplinary team and promote interpersonal skills.
- Facilitate seamless service between the Trust and local services.
- Promote a healthy lifestyle for patients & staff in line with national strategy requirements.
