Neighbourhood Health - Care Coordinator
This is an exciting opportunity to join Runcorn Primary Care Network (PCN) and be part of the start of our new Neighbourhood Health team, employed by our local federation, GPHC. The initial focus will be on developing a new Neighbourhood Multidisciplinary Team (MDT) approach, bringing together professionals from multiple health, care and community organisations around the needs of local people.
