As a Community Nurse within our 019 Healthy Child Programme, you willplay a key role in shaping how we support children and families to thrive.

Working as part of an innovative Family Hub model, you will focus onstrengthening the home learning environment, improving engagement with families and increasing attendance and outcomes from developmental reviews.

This is a highly collaborative role, where you will build strong relationships with Specialist community public health nurses, early years settings, education and wider partners to ensure children are identified earlyand supported through the right pathways at the right time. You will bringcuriosity, initiative and a passion for improving outcomes, helping to reduceinequalities, address missed opportunities and support more children to achievea Good Level of Development.

To support delivery of the Healthy Child Programme using evidence based practice to activelypromote the health of children and young people.

To be a named /link nurse for designated schools/ early years settings whilst holding acaseload which incorporates the 4 levels of intervention (Community, Universal,Universal Plus and Universal Partnership Plus).

To deliver andengage in health promotion programmes and public health initiatives to preventill health, early identification of abnormalities and work with all clients inthe 0-19 service and communities to reduce health inequalities as identified within individual Health Plans.

To conduct health needs assessments in the home, school, clinics or community settings in a planned programme of care within a variety of settings.

Offer appropriate interventions, delivering agreed programmes of care and evaluating the outcomes for the child, young person and family.

Plan and implementschool entry assessments delegating duties as appropriate.

As a registered nurse, the post holder will work within the 0-19 integrated public health nursing Team to deliver a flexible 8am-8pm service providing access to the Healthy Child Programme for children, young people and families accessing the 0-19 Service.

The post holder will have delegated responsibility for effective early help and the assessment of Health needs, implementation and evaluation of care as part of the Healthy Child Programme, at community, Universal, Universal Partnership and Universal Partnership Plus level.

This will include children with complex health needs; children/young people who may be presenting with a cause for concern, subject to safeguarding and Children in Care; parents/young people who are experiencing emotional health needs.
