Complex Discharge Coordinator

We are looking for a Registered Nurse or an Allied Health Professional with leadership skills and experience of discharge management who will be responsible for maintaining patient capacity and flow throughout the system focusing on effectively managing timely and safe discharges from all Community Inpatients areas which include 6 different sites across Northamptonshire. The post holder will be part of an integrated multi professional inpatient team that includes senior managerial colleagues, therapists and medical staff & wider trust and social care colleagues.

The post holder will work closely with external health organisations and other care providers such as but not limited to intermediate care team, mental health teams, community nurses and nurse specialists to develop discharge plans for patients requiring ongoing support in the community.

The post holder will be expected to independently manage their own caseload, deputise for the Flow Lead where required including data collection & data reporting of both admission & discharge information. The post holder will provide support to junior colleagues and assist them in problem-solving any issues relating to patient flow.

The post holder will have regular contact with external colleagues using a variety of communication methods including video and telephone conferencing, email and telephone and must have excellent communication and IT skills.

The role may involve some weekend working on a rota basis.

Main duties of the job
  • To be proactive in reducing delayed transfers of care, to ensure patient information is accurate, consistent and completed promptly.
  • To work in collaboration with internal and external multidisciplinary agencies to develop clear safe discharge plans.
  • To support the development of processes, policies and practices that ensures all relevant assessments, services and resources are available to support safe discharge.
  • Demonstrate working knowledge and practical application of legislation relevant to discharge and transfer of care including the Care Act.
  • Excellent interpersonal skills and the ability to utilize them to communicate effectively with internal and external system partners
  • Main point of contact for patients and families to advise and guide them through complex discharge processes and support patients and families through challenging and stressful situations
  • Ability to manage conflict and de‑escalate situations in relation to discharges and support discharge coordinators with difficult conversations on a daily basis.
  • To maintain database of patient delays, data inputting and provision of daily information updates, with excellent IT and data analysis skills
