Plas Meddyg Surgery in North Wales is a dynamic and patient‑focused organisation.
We are looking for an enthusiastic and highly motivated Practice Nurse who believes in a vision for high‑quality clinical care and compassion within Primary Care.
The successful applicant should ideally already have experience of long‑term conditions and working in Primary Care or General Practice.
The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.
The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and defined competencies.
The main duties of the role include (but are not limited to):
- Undertake chronic disease management clinics, including Asthma, COPD/Spirometry, Diabetes and Chronic Heart Disease.
- Identify and manage appropriate treatment plans for patients at risk of developing a long‑term condition.
- Perform basic nursing tasks such as blood pressure monitoring, ECGs, phlebotomy, etc.
- Participate in vaccination and immunisation programmes; administer injections, ensure safe vaccine storage and implement cold chain policy.
- Undertake health screening, surveillance and new‑patient screens.
- Take pathology samples for analysis (blood, urine, cervical samples, etc.).
The role will require flexibility to meet the demands of this large practice and an individual work plan will be agreed with the successful applicant.

Person specification- Registered general nurse, active statutory registration.
- Recognised qualification to diploma level or above in areas of chronic disease management.
- Competent in basic nursing duties such as wound management, ear care, vaccinations, venepuncture.
- Knowledge of needs of patients with long‑term conditions.
- Knowledge of patient group directions and associated policy.
- Experienced with GP clinical IT systems.
- Demonstrable commitment to professional development.
- Excellent communication and interpersonal skills.
- Understanding of professional confidentiality.
- Flexible, reliable, and responsive to change to meet the needs of the service.
- Understanding of QAIF and the GMS Contract.
- Ability to prioritise workload and work under pressure.
- Ability to adapt to a changing and demanding environment.
- Experience of community or primary care nursing.
- Ability to speak Welsh.
