Position Overview Outpatient primary care role serving a diverse patient population Practice within a multi-site primary care system across central Vermont Opportunity to provide comprehensive, longitudinal care focused on prevention and wellness Minimal after-hours call shared across sites (triage only) Supportive team-based care model with strong clinical infrastructure

Clinical Responsibilities Provide comprehensive primary care services to patients of all ages Promote preventive care, wellness, and chronic disease management Empower patients to actively participate in their health care Provide appropriate follow-up and clear patient communication Refer patients to specialty care as clinically indicated Provide clinical supervision to advanced practice providers and nursing staff Participate in quality assurance and quality improvement initiatives Engage in state and national disease collaborative efforts

Qualifications MD or DO Board certification strongly preferred Eligible for licensure in the state of Vermont Completion of accredited medical education and residency training
