ABOUT US:

Community Health is a multi-site FQHC serving 80% of the primary care needs in our community (central Vermont). Our mission is to service our patients with quality, efficiency, accessibility and equity. As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state and regional health care environment.

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.



POSITION SUMMARY:

Community Health is seeking an Express Care Physician to treat urgent care patients at our clinic in Castleton, Vermont. Physician candidates are expected to be knowledgeable and skilled in the delivery of quality care. Responsibilities include, but are not limited to:

  • Provide appropriate patient follow-up and communicate effectively with patients.
  • Refer patients to primary and specialty care as necessary.
  • Provide clinical supervision to mid-level providers and nursing staff.
  • Participate in quality assurance activities.
  • Procedural skills, e.g. suturing, I&D, reading EKG and x-ray, etc.
  • Participate in State and National disease collaborative efforts.


KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:

  • Previous Experience in Urgent Care or Emergency Room Services preferred.
  • Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
  • Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
  • Quality-focused team player.
  • Excellent organizational, time management and problem-solving skills.
  • Ability to function effectively as part of a multi-disciplinary team.
  • Effective oral and written communication skills.
  • Commitment to providing exceptional patient care.
