LVN​/LPN – LTC​/SNF

Job Summary:
Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure the delivery of high-quality patient care while adhering to regulatory and facility standards.

Responsibilities:

• Administer nursing procedures and medications accurately and in a timely manner.
• Monitor residents’ health status, document findings, and report any significant changes.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Maintain compliance with facility policies, state, and federal regulations.
• Ensure proper use of PPE, including wearing a photo badge and bringing necessary identification.
• Follow infection control protocols, including COVID-19 vaccination requirements and exemptions.

Qualifications:

• Valid LVN/LPN license and current state licensure.
• Prior nursing home and PCC experience.
• Ability to work independently and as part of a team.
• Strong communication and observational skills.
• Must have completed or be willing to complete COVID-19 vaccination or approved exemptions.
