Position: Transition Care Program - Coordinator AHP2

Northern Adelaide Local Health Network – Northern Adelaide Geriatric Service – Modbury Hospital - Modbury Salary: $91,785 - $106,330 per annum, (pro rata) plus Superannuation and Salary Sacrifice benefits AHP2 – Multiple Ongoing/Temporary Part-time positions available, Part-Time – 26.25 - 30 hours per week Note

About the Role

Working within our modern facilities, you'll collaborate with multidisciplinary teams to ensure seamless care transitions, reducing hospital readmissions and improving patient outcomes across Adelaide's northern communities.

What You'll Do
  • Plan, implement and evaluate tailored care plans for patients leaving hospital.
  • Conduct thorough assessments to identify appropriate support services.
  • Monitor patient progress and adjust services as needed.
  • Maintain accurate documentation and reporting requirements.
  • Apply advanced clinical judgment to manage complex cases and discharge planning.
  • Liaise with health professionals, service providers and families to ensure holistic care.
  • Contribute to quality improvement, research and service development initiatives.

To be eligible for this position, you must meet the minimum qualification requirements as aligned to the relevant Allied Health specialty, as defined in the relevant role description.
