Job DescriptionIn this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed by a team of Registered Nurse (RN) Case Managers and Licensed Vocational Nurses (LVNs). You will serve as the first point of escalation, provide timely support, and develop and implement nursing educational programs to promote ongoing staff development and patient satisfaction.
You will also meet regularly with customers to maintain relationships and provide updates such as readmission reports.
Hybrid work:
- Sacramento Office – Spend 2 days per week in the office for administrative and team management duties.
- Field work – Spend 3 days per week (average 12+ visits per week) in the field conducting supervisory visits and offering direct support to RN Case Managers.
- Oversee clinicians in patients’ homes.
- Handle incoming calls promptly and coordinate follow‑up actions.
- Coordinate training and educational programs to improve clinical outcomes.
- Assess staff learning needs and formulate specific plans and goals.
- Act as the first point of contact for field clinicians with escalation questions.
- Coordinate bi‑weekly Case Conferences to discuss patient progress and revise care plans.
- Review and evaluate each case, review services, discuss impressions, and guide clinicians toward effective performance and quality care.
- Review patients’ clinical diagnosis, medications, procedures, and clinical course.
- Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care.

- Two years’ management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
- Graduate of an accredited school of nursing;
Bachelor’s Degree preferred. - Current California Registered Nurse (RN) license.
- Current Basic Life Support (BLS) card.
- Valid California driver’s license with acceptable driving record.
- All licenses and certifications must be current at time of hire and sustained throughout employment.
