The RN Case Manager is responsible for the comprehensive management of patients within an assigned home‑health caseload. • Manage a designated caseload of home‑health patients
• Provide direct nursing care and conduct in‑home assessments
• Develop, implement, and evaluate individualized care plans

• Monitor visit utilization and support cost‑effective care delivery
• Maintain accurate, timely clinical documentation
• Participate in orientation, competency evaluations, and staff consultation as needed
