The RN Case Manager is responsible for the comprehensive management of patients within an assigned home‑health caseload. This includes direct patient care, care planning, interdisciplinary coordination, and ensuring compliance with regulatory standards. The role emphasizes quality outcomes, efficient visit utilization, and strong patient‑family communication.
Key Responsibilities- Manage a designated caseload of home‑health patients
- Provide direct nursing care and conduct in‑home assessments
- Develop, implement, and evaluate individualized care plans
- Coordinate care with physicians, therapists, social workers, and other team members
- Monitor visit utilization and support cost‑effective care delivery
- Maintain accurate, timely clinical documentation
- Participate in orientation, competency evaluations, and staff consultation as needed
- Ensure adherence to all regulatory, quality, and safety standards
- Home Health Case Manager experience (must‑have)
- Strong clinical assessment and critical‑thinking skills
- Ability to work independently in the field
- Excellent communication, organization, and time‑management abilities
- Experience coordinating interdisciplinary care
