Nurse Practitioner​/Physician Assistant - Sacramento, CA

We are seeking a dedicated and patient‑centered Nurse Practitioner or Physician Assistant to provide high‑quality outpatient primary care within a community health clinic environment. • Provide comprehensive primary care services for patients of all ages

• Conduct physical exams, health assessments, and diagnostic evaluations

• Diagnose, treat, and manage acute and chronic conditions

• Prescribe medications and develop treatment plans within scope of practice

• Coordinate patient care, referrals, and follow‑up services

• Maintain accurate, timely, and compliant medical documentation

• Collaborate closely with physicians, nursing staff, and care teams

• Active and unrestricted California NP or PA license

• Board certification (NP or NCCPA)

• 1–2 years of clinical experience preferred

• Experience in community health is a plus

$131,040 – $170,352 annually (based on experience)
