Choice Health at Home is seeking a PRN Physical Therapist to provide home health services in Safford, AZ. You will evaluate patients, design personalized treatment plans, and work with a multidisciplinary team to restore function and improve quality of life in patients' homes.
The role requires an accredited PT degree, valid PT license, CPR certification, and a valid driver's license. Prior home health and Home Care Home Base experience are preferred for this PRN position.
