• Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
• Perform independently under stress and in emergency situations.
• Drive between locations, navigating with GPS or other tools.
• Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility, and other personnel involved in patient care.
• Assess health status by obtaining relevant health and medical history; perform a physical exam based upon the patient’s age and history; identify significant health and medical risk factors.
• Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient clinical problems and healthcare needs; develop a diagnosis by analyzing collected data.
• Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges; prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
• Develop a treatment plan in collaboration with the patient and significant others, including Medicare, CMS, ancillary services, and treatment; collaborate with nursing, medical, and other healthcare team members to ensure quality patient care.
• Refer patients to providers as appropriate for consultation or specialized health resources and treatment; ensure continuity of care by acting as a liaison among the multidisciplinary care team.
• Supervise nurse practitioners and physician assistants per the Collaborative Agreement.

• Doctor of Medicine or Osteopathy required.
• Current unrestricted State license to practice medicine.
• Prior supervisory/management experience.
• Demonstrated interpersonal, supervisory, and management skills with effective communication.
• Proven organizational development and project management abilities.
• Knowledge of regulations and reimbursement in home health care, with proven leadership abilities.
• Ability to sit, stand, bend, lift, and move intermittently; lift at least 50 lbs.
• Current and valid driver’s license; reliable transportation and valid automobile insurance coverage.

• Board certification in family practice, internal medicine, geriatric, or palliative care preferred.
• Preferred 2 years of experience in healthcare with a combination of acute care, geriatric, and/or long‑term care practice.
• Experience with geriatric patients preferred.

• Home Office: 4760 Fashion Square Blvd, Saginaw, MI 48604
• Main Service Area:
  Saginaw and surrounding region; patient services in facilities and private homes.

• Office hours: 8:00 AM – 5:00 PM, Monday through Friday.

$150,000 – $200,000 USD
