Full Time Registered Nurse. Territory:
Saginaw, MI. Optimal Care is seeking a Full Time Registered Nurse committed to providing the highest quality of care.

We are looking for highly professional, self-motivated RNs who love working with the adult and geriatric homebound population. Home care experience is preferred but not required.

Registered Nurse Responsibilities:

  • Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatments signed by the physician.
  • Provides initial and ongoing comprehensive assessments of the client's needs.
  • Initiates diagnostic, preventative, and rehabilitative nursing procedures as appropriate to the client's care and safety.
  • Observes signs and symptoms and reports to the physician and/or appropriate health professional as often as needed.
  • Teaches, supervises, and counsels the client and family regarding home nursing procedures and other care needs as appropriate to the client's condition.
  • Coordinates the total plan of care and maintains continuity of care by liaising with other health professionals assigned to the same clients.
  • Assigns home health aide services to specific patients.
  • Supervises LPN and paraprofessionals providing services.
  • Develops, prepares, and maintains individualized client care progress records with accuracy, timeliness, and according to policies.
  • Responsible for following all policies and procedures regarding service delivery, documentation, and care coordination with full compliance.
  • Participates in in-service programs and quality improvement initiatives.
  • Plans for client discharge from services.

Qualifications:

  • Minimum of associate's degree in nursing by accredited program
  • Registered Nurse License recognized by the state in which practicing
  • Minimum of 1 year experience working as a Registered Nurse preferred
  • Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
  • Demonstrates knowledge of the principles of growth and development over the life span
  • Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
  • Demonstrates computer skills sufficient to use the electronic medical record effectively
  • Valid Driver's License
  • Current Auto Insurance
  • Reliable Transportation
