Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings. The Nurse Practitioner will function in collaboration with physicians according to a collaboration arrangement. The Nurse Practitioner practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
- Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
- Determining follow‑up assessments according to patient need and best‑practice guidelines.
- Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care.
- Assessing health status by obtaining relevant health and medical history.
- Performing a physical exam based upon the age and patient history as per the standard care arrangement with a collaborating physician.
- Ordering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs.
- Performing diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges.
- Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency.
- Developing a treatment plan in collaboration with the patient and caregivers.
- Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment.
- Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.
- Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.
- Holding case conferences with families when appropriate to advance the care plan.
- Meeting minimum productivity requirements based on practice standard.
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- Master's or doctoral degree as a Board‑Certified Nurse Practitioner.
- Current State License as a Registered Nurse and Nurse Practitioner.
- Nursing skills as defined by accepted nursing standards.
- Ability to sit, stand, bend, lift, and move intermittently.
- Must be able to lift at least 35 lbs and bear the weight of an average adult effectively.
- Current and valid driver’s license.
- Reliable transportation and valid automobile insurance coverage.
- Demonstrates ability to assess and respond to the needs of patients, families and caregivers in skilled‑nursing facility settings.
- Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
- Ability to handle multiple priorities, documentation requirements and deadlines.
- 2 years clinical experience in a post‑acute or as a Nurse Practitioner preferred.
- Experience with geriatric patients preferred.
- Familiarity with Aprima EMR.

- Office hours: 8:00 AM – 5:00 PM, Monday through Friday.
