Overview

The Patient Placement Coordinator RN is responsible for accurately applying admission criteria for patients with a physician requesting an initial bed assignment to ensure timely and appropriate status and room placement. This involves interaction with physicians, nursing staff, Medical Social Work, community resources, patients and families. The role includes applying medical necessity criteria and conducting record reviews, and practicing in accordance with the care management process using tools, standards, models, goals and objectives, and performance improvement concepts.

Active participation in team meetings and education of staff is a key part of the role.

Responsibilities
  • Performs medical necessity assessment to ensure accurate and timely patient status and bed assignment using hospital selected medical necessity criteria such as Inter Qual (50%).
  • Actively educates the health care team and physicians regarding the appropriate patient status and bed assignment (30%).
  • Completes all documentation and data entry requirements in EPIC (10%).
  • Communicates with outside agencies to ensure accurate information is received to evaluate if transfers are appropriate prior to patient arrival (10%).
  • Other duties and responsibilities include using critical thinking in all interactions, setting priorities, explaining processes when patient condition does not match UR criteria, maintaining positive interactions with staff and physicians, and building professional relationships with patients, families, facilities and resources.
  • Assigns bed placement according to medical necessity for surgical admissions, transfers in-house and to other affiliates.
  • Performs early intervention with review of potential Inpatient and Observation patients using all available test results within EPIC.
  • Offers to help peers during high volume cases and accepts accountability for clinical outcomes that the role can influence.
  • Anticipates scheduling challenges during vacations, holidays and weekends and manages workflow to avoid delays.
  • Provides excellent customer service to patients, physicians, office staff, and medical staff requiring patient placement and delegates tasks appropriately to the Patient Placement assistant.
Qualifications
  • Education: Associate degree (ASSOC) required;
    Bachelor’s degree in Nursing preferred. Incumbents have until 1/1/2025 to obtain a Bachelor’s degree in nursing.

  • Licensure/

    Certifications:

    RN (Registered Nurse). Certifications include TEAM ADVANCED:
    Techniques for Effective Aggression Management;
    Equivalent experience in related qualifications (MI as Qualification Source). Essential: true.
  • Experience: Recent hospital experience in ICU, CCU, or strong Med/Surg. Knowledge of Commercial and Government Payer guidelines and ability to communicate payer guidelines and their impact on patient status and bed assignment. Essential: true.
  • Skills: Organizational, analytical, writing and interpersonal skills; critical thinking, problem solving, and deductive reasoning; knowledge of pathophysiology and disease processes; basic computer and courteous phone skills; competency with Microsoft Windows.
Physical/Working Conditions
  • Exposure to stressful situations including trauma, grief and death; ability to wear PPE as required.
  • Must be able to move about the facility, perform the functions of the job, and have overall vision and hearing with or without assistive devices.
  • Frequent sitting, standing, and walking; may require postural changes; occasional lifting up to 11-20 lbs, with greater lifting for shorter periods depending on task.
  • Possible exposure to blood borne pathogens and other potentially infectious materials; comply with health and safety protocols.
