Wellspring Lutheran Services is a statewide nonprofit dedicated to serving seniors, families and children throughout Michigan. We are currently seeking a full-time Nurse Supervisor to join our nursing team at Covenant Skilled Nursing and Rehab at Wellspring in Saginaw.

Covenant Skilled Nursing and Rehab at Wellspring is a partnership between Covenant Health Care and Wellspring to help patients transition from the hospital to home. The new facility is part of a $5 million investment by Covenant and Wellspring to provide the most advanced post‑acute care and services in the Great Lakes Bay Region. The facility also provides physical, occupational and speech therapies performed by Mary Free Bed therapists, working closely with the nursing department to ensure the highest quality in post‑acute programming.

As a Nurse Supervisor with Wellspring, you will:

• Provide quality nursing care to residents in an environment which promotes their rights, dignity, and person‑centered care
• Provide individualized attention, encouraging each resident’s ability to maintain or attain the highest level of independence and well‑being
• Assist in maintaining a safe, neat, and clean environment; report deficiencies as appropriate
• Protect residents’ personal belongings and promptly report missing items per policy
• Keep nursing leadership informed of resident status and care needs
• Communicate effectively with residents, families, visitors, peers, and supervisors
• Participate in meetings and assist with staff education and orientation as requested
• Lead all staff on assigned shifts, reporting to the DON/Nurse Manager as needed
• Evaluate care outcomes daily to ensure high‑quality standards
• Collaborate with the MDS Coordinator regarding ADL scoring and care documentation
• Communicate and monitor staff performance, following disciplinary steps as needed
• Complete evaluations for nursing staff on the unit as directed
• Ensure thorough walking rounds and shift reports between aides and nurses
• Review EHR documentation regularly to ensure accuracy and timeliness
• Ensure documentation reflects residents’ progress, maintenance, or decline, noting unavoidable circumstances as required
• Complete and review incident reports; assist DON with follow‑up and staff guidance

• Current RN or LPN license issued by the State of Michigan Board of Nursing
• Successful completion of an accredited nursing program
• Minimum of 2 years of nursing experience; home health, medical, surgical, or critical care experience preferred
• Strong leadership, communication, and organizational skills
• Compassionate, dependable, and committed to quality resident care

• Pay range: $36.00 – $38.25 / hour based on experience and licensure (RN or LPN)
